Enrollment Process
When applications are approved, parents are notified and will need to complete the following to enroll and secure a place for their child.
This will require:
- Completion of registration, covenant statements, and financial agreement forms.
- Payment of school fees.
- Submission of physical examination form (#3040) and immunization records (HRS #680). These forms are available from a local doctor and are required by the state of Florida.
- A copy of student's birth certificate.
- A copy of student's social security card.
- School release form from previously attended school.
