Enrollment Process

When applications are approved, parents are notified and will need to complete the following to enroll and secure a place for their child.

This will require:

  1. Completion of registration, covenant statements, and financial agreement  forms.
  2. Payment of school fees.
  3. Submission of physical examination form (#3040) and immunization records  (HRS #680).  These forms are available from a local doctor and are required by the state of Florida.
  4. A copy of student's birth certificate.
  5. A copy of student's social security card.
  6. School release form from previously attended school.
