Admissions Forms
Students may not start school until all state required documents listed under Admission – Clinic Forms tabs are on file at SCS. Please make sure you’ve turned in the necessary documents.

Required Immunizations
All new students must have the required immunizations for their grade level and submit a DH680 or DH681 form. Returning students in grades K and 7 have an additional immunization needed at those grade levels and will need an updated immunization record. (Forms are provided by doctor.)
Religious Exemption Form (optional)
Families wishing to apply for a religious exemption from vaccinations must make and appointment with Sarasota County for approval.

Campus Safety
The well-being of our students is a top priority and below are a couple of important ways we help keep our campus safe.
Parents & Visitor Check-In
All parents, volunteers and visitors to our campus are required to have their driver’s license scanned at the front desk in order to obtain a visitor/volunteer badge to be on campus.

Prescription Medication Form
Students may not carry prescription or over-the-counter medication with them at school. All medication must be registered in the clinic.
Non-Prescription Medication Form
A variety of non-prescription medications are available in the clinic for your convenience, however all students must have this form completed.
Over the Counter Medication Form
This form is for non-prescription items that are not provided by the school. Students may not carry over-the-counter medication with them at school. All medication must be registered in the clinic.

Custom spirit wear can be ordered individually through our Sideline Store (click below). Please note that all items ordered from the Sideline Store (excluding outerwear) are considered spirit wear and can only be worn on Fridays.
